Online Medical Records Request

When submitting a request online, please complete all required fields. You will need to upload a copy of your government issued photo ID.
Requests will be processed within five to ten business days of receipt. Requests are fulfilled by our medical records partner, Verisma.

Raleigh General Hospital now offers medical records requests online.
To obtain your medical records you will need to complete an authorization form that gives Raleigh General Hospital permission to release your medical records. You may fill out a form at the hospital or print a Request of Information form and bring it to the hospital.
When you bring it to the hospital please bring it to the main front entrance information desk. You be required to show proof of identification. If you are picking up someone else records, you will need:
- Letter from the patient
- Copy of a Medical Power of Attorney
- Letter of Administration for deceased patients
All records will be mailed out within 7-10 business days.
