Advance Directives


Raleigh General Hospital recognizes that patients have the right to participate in decision making regarding medical care, including the right to refuse life-sustaining treatment.

Advance Directives include two specific alternatives, directed by the patient. They are:

Medical Power of Attorney - a written statement that gives legal authority to another person (agent) to make health care decisions on your behalf when you are unable to do so due to illness or injury.

Living Will - a written statement that describes your personal wishes about how much treatment is desired in case of terminal illness.

In compliance with Federal law, patients will be given information and assistance as necessary to complete Advance Directives if the patient or surrogate so chooses. You should have received a brochure on Advance Directives when you were admitted.

If you did not receive one, please contact the Admitting Office at 256-4286 or the Social Services Department at 256-4364.
